Our polices and practices

Insurance

ClearSkin Dermatology accepts the following insurance plans as well as self-pay. If you do not see your plan below or should you have questions regarding your plan benefits and eligibility, feel free to call our office for assistance. We are on several plans, but it is the patient's responsibility to check their provider book to make sure we are participating in your specific plan.

What You Should Bring

Please bring a photo ID and your insurance card to the appointment. We may also need the social security number of the guarantor so we can file the insurance claim, and failure to bring this may result in the full cost of the appointment at the time of service.

Cancellation/No-Show Policy

If you must cancel your appointment, please give us at least 24 hours in advance notice so that time can be given to a patient with an immediate problem. You will be considered a no-show if you miss an appointment and do not notify us in advance.  There is $75 charge for no-shows (or cancellation within 24 hours of your scheduled appointment time, $150 for surgical/cosmetic procedures)  and payment of this fee must be made before further appointments are allowed. We will make every effort to remind you of your appointment via email, phone and/or text, but please help us by always making sure your phone numbers and email address are updated with us during visits. 

Payments

Payment, including coinsurance, patient balances, and deductibles will be collected at the time of service. We accept most forms of payment including cash, check, Visa, Master Card, and American Express. We also have a credit card on file (CCOF) policy to allow for smoother transactions. The credit is stored safely and held on a secure site with our third party merchant service processor, Square. Your credit card information will not be held in our office electronic health record so no financial information will be attached to your file. 

Prescriptions

At the time of your visit you may be provided with prescriptions to take to your pharmacy to fill. If you find the cost to cover your prescription is too high, please call our office to see if there is an appropriate less expensive option. If a prior authorization is needed, this may take up to two weeks. Should you have any questions about your prescriptions please refer to the medication instruction sheet provided to you at the time of your visit or call our office for assistance.

Prescription Refills

If you would like a prescription refill, please have the pharmacy fax us a request at 407-866-1181 and allow up to 72 hours for it to be processed.

Lab Tests

It is sometimes necessary to order lab tests in order to obtain additional information about your condition and/or to monitor the effects of treatment. If lab work is required prior to a return visit to our office, we do ask that you complete it at least 48 hours in advance to allow ample time for the lab to process and send results to our office. It is your responsibility to ensure that the results of any lab testing are received by our office prior to your scheduled appointment. Results may be faxed to us or you may bring them with you. 

Medications

We ask that whenever you come in for an appointment that you bring a list of all current medications. 

Minors

A parent or the legal guardian must attend the initial appointment with the child and all appointments involving invasive treatment. Minor children must be accompanied by a parent or authorized individual 18 years of age or older for all appointments. In the event a parent or legal guardian cannot bring the child to a routine appointment, we need a specific form (the Parental Temporary Assignment of Authorization to Treat Minor Child) signed but the signature must be witnessed by a staff member of ClearSkin Dermatology.
